Reassessment: vagus nerve stimulation for epilepsy.

چکیده
A July 1997 assessment of the American Academy of Neurology (AAN) Therapeutics and Technology Subcommittee reviewed vagus nerve stimulation (VNS) for epilepsy, and concluded that it was promising, but not yet established. Subsequent to that review, a second multicenter, randomized, controlled clinical trial has shown safety and efficacy of VNS for treatment of intractable partial seizures. Given the importance of this novel therapy for epilepsy, and the new information provided by a second major study, the Therapeutics and Technology Subcommittee requested an update on VNS. Use of VNS in epilepsy has recently been reviewed. VNS uses intermittent stimulation of the left vagus nerve in the neck to reduce the frequency and intensity of seizures. Mechanism of action of VNS remains uncertain, but stimulation does not induce grossly visible alterations in the human EEG. Recent studies suggest that metabolic activation of certain thalamic, brainstem, and limbic structures may be important in mediating the effect of VNS. Depletion of norepinephrine in the locus coeruleus attenuates the antiseizure affect of VNS. After open-label studies, sponsors of VNS engaged in a multicenter randomized study, named E03, details of which were reviewed in the previous assessment. Results of the trial were positive, and the technology was considered promising, but the number of patients receiving VNS was insufficient to achieve Food and Drug Administration approval. Therefore, a second randomized, controlled, multicenter clinical trial, called E05, was undertaken.

Vagus nerve stimulation in the treatment of nervous system disease: a review article
The vagus nerve (VN), the longest cranial nerve and an essential part of the parasympathetic system, connects the central nervous system to respiratory, cardiovascular, immune, gastrointestinal, and endocrine systems and is involved in the maintenance of homeostasis by controlling these systems. Vagus nerve stimulation (VNS) is related to any method that would stimulate the vagal nerve via elec...
متن کاملNeurostimulation as a Putative Method for Treatment of Drug-Resistant Epilepsy in Patient and Animal Models of Epilepsy
A patient with epilepsy was shown to have neurobiological, psychological, cognitive, and social issues as a result of recurring seizures, which is regarded to be a chronic brain disease. However, despite numerous drug treatments, approximately, 30–40% of all patients are resistant to antiepileptic drugs. Therefore, newer therapeutic modalities are introduced into clinical practice in which invol...

Objectives:Vagal pathways in gastrointestinal tract are the most important pathways that regulate ischemia/reperfusion (I/R). Gastrointestinal tract is one of the important sources of melatonin production. The aim of this study was to investigate probable protective effect of the interaction between vagus nerve and melatonin after I/R. Materials and methods:This study was performed in male rat...

Vagus Nerve Stimulation Electrical stimulation of the 10th cranial nerve or vagus nerve stimulation (VNS) has become a valuable option in the therapeutic armamentarium of adults and children with refractory epilepsy. Since its introduction in 1989 over 50,000 patients have been treated with VNS worldwide. Efficacy, side effects and tolerability have been extensively studied. The precise mechani...

PURPOSE OF REVIEW The purpose of this review is to provide an evidence-based update on the neurostimulation options available for patients with drug-resistant epilepsy in the United States and in European countries. RECENT FINDINGS The field of neurostimulation for epilepsy has grown dramatically since 1997, when vagus nerve stimulation became the first device to be approved for epilepsy by t...
